Checklist

Complete these 7 (seven) steps to ensure your child’s enrollment into the camp(s) you are requesting. All steps must be completed in order to enroll your child.


  • 1 Camper Info Complete 1 (one) application per child. Applications may be duplicated for additional applicants.

  • 2 Camp/Week Choices Circle the correct week number(s) for each week of camp you’re applying for. Example: If you want to apply for camps during weeks 2, 5 and 8, then circle "2", "5" and "8", and not "3" (as in "three weeks total")!

  • 3 Before/After Camp Care Choices Circle all of the Before and After Camp Care weeks (and their locations, if required)

  • 4 Payment Include a $25 non-refundable, non-transferable deposit per child, for each week of camp you’re applying for. If attending a Before or After Camp Care, include a $5 non-refundable, non-transferable deposit per child for each week of Before and/or After Camp Care you are applying for. These fees are part of the total fee for the week and not additional charges. Checks, money orders and all major credit cards are accepted.
    Examples:
    2 weeks of camp for 1 child = $50 deposit
    2 weeks of camp each for 2 children = $100 deposit

  • 5 YMCA Membership If your camper is not a YMCA member, include payment for YMCA membership. See Application page iv for details.

  • 6 Return Application Mail or hand-deliver your completed application(s) —one per child—to the branch which will be hosting the camp you requested (not to any of the “additional sites”! Example: If you are applying for a Specialty Camp held at the Pontifical College Josephinum, you would send your application to either the Liberty Township/ Powell YMCA or the North YMCA, not the Josephinum. See page 2 for camp sites and additional locations.

  • 7 Health Forms In order for your child to be officially enrolled in camp, we must receive ALL OF YOUR COMPLETED HEALTH FORMS. You can fill them out electronically, then print them and mail or bring them to your camp YMCA branch. You can also call your branch to request that a packet be sent to you.

    That's it...You're done! We'll see you at camp!

    Parent Orientations

    Parent Orientations are held prior to Week 1 of camp. Each family is invited to attend. Call your branch, or email us to find out the times.
